AFMC has notified members
of the Arkansas Physical Therapy Association that under the Medicaid
Guidelines sent out in December 2002 progress notes must be signed
each date of entry with a full signature and credentials. AFMC is currently denying anything that does not contain a full
signature (i.e. an initial for the first name and signature for the
last).
